The Lingual Sage clasp is used on molars and bicuspids when they are fully erupted. It gains its retention by engaging the undercuts of the proximal surfaces and the lingual contour to the center of the tooth. Because this clasp does not have any part crossing the occlusion, it is often used in situations where the occlusion is extremely tight and any wire crossing over it would create an occlusal interference.
It is mainly used as part of a final retainer and is not recommended for active appliances as it may not provide sufficient retention once the appliance is activated.
